ABSTRACT

Resumen El paro cardiorrespiratorio (PCR) en la mujer embarazada es una situación infrecuente, grave y de manejo multidisciplinario. La reanimación cardiopulmonar requiere consideraciones y particularidades propias de la embarazada, centradas en la fisiología y la anatomía, teniendo especial consideración la compresión aortocava, la intubación-ventilación difícil, la presencia de estómago lleno y el hecho que hay dos vidas involucradas. Las principales causas de PCR son las hemorrágicas, seguidas de las embólicas, cardiovasculares, anestésicas e infecciosas. Las principales acciones incluyen activación del código azul obstétrico con respuesta rápida para una eventual realización de histerotomía de emergencia oportuna en el mismo sitio evitando el traslado al quirófano, compresiones torácicas de buena calidad, desviación manual uterina a la izquierda, intubación endotraqueal y manejo avanzado de la vía aérea, todo esto con el fin de mejorar la sobrevida materno-fetal. La cesárea perimortem es un pilar en el manejo, favoreciendo el desenlace materno y eventualmente el fetal. Se debe realizar a los 4 minutos de una reanimación cardiopulmonar no exitosa. Sin embargo, aún hay retardo a la hora de indicarla, por lo que se debe incentivar el entrenamiento, la simulación en resucitación cardiopulmonar materna y las guías clínicas para todo el personal involucrado en la atención de pacientes obstétricas.


Abstract Cardiopulmonary arrest is a rare event during pregnancy and labor. It involves many subspecialties and allied health providers. Besides it requires knowledge of maternal physiology as it relates to resuscitation, particularly aortocaval compression, difficult airway, full stomach and the fact that there are two lives involved. The most frequent causes of cardiac arrest during pregnancy include bleeding, followed by embolism, infection, anesthesia complications and heart failure. The main steps required are: obstetric code activation with appropriate response for performing timely emergent hysterotomy in the same place avoiding the transfer to operating room; good-quality chest compressions; manual uterine displacement to the left, advanced pharmacological and airway management; and optimal care after resuscitation to improve maternal and fetal outcomes. Although current recommendations for maternal resuscitation include the performance of perimortem cesarean section after four minutes of unsuccessful cardiopulmonary resuscitation, deficits in knowledge about this procedure are common. Therefore, training and available evidence-based guidelines should be put in place for all obstetric caregivers.

ABSTRACT

RESUMEN El síndrome de Eisenmenger es una patología del sistema vascular pulmonar, que se caracteriza por un incremento de las resistencias en los vasos sanguíneos pulmonares, en pacientes con antecedentes de cardiopatía con comunicación entre la circulación pulmonar y circulación sistémica. Este síndrome en la mujer embarazada representa un alto índice de morbilidad y mortalidad tanto para la madre como para el feto; los síntomas que presentan surgen como consecuencia de los cambios fisiológicos en el sistema cardiovascular y hematopoyético. Es un desafío para el obstetra el manejo de estas pacientes idealmente debe ofrecerse asesoría preconcepcional, y valoración por cardiología; si no es posible, se realizará evaluación del riesgo desde el primer control prenatal. El mejor tratamiento consiste en oxigenoterapia, medicamentos vasodilatadores, y anticoagulantes, logrando disminuir el porcentaje de pobres desenlaces materno-fetales. Es de vital importancia definir el momento del nacimiento con el fin de tomar las medidas terapéuticas necesarias para evitar complicaciones, además se requiere de un manejo multidisciplinario, incluido el apoyo por una unidad de cuidado intensivo dado la alta probabilidad de complicaciones asociadas a esta patología.


ABSTRACT Eisenmenger syndrome is a pathology of the pulmonary vascular system, which is characterized by an increase in resistance of the pulmonary blood vessels in patients with a history of heart disease with communication between the pulmonary circulation and the systemic circulation. This syndrome in pregnant women represents a high morbidity and mortality rate for both the mother and the fetus. The symptoms that arise are a consequence of the physiological changes in the cardiovascular and hematopoietic system. The management of these patients is a challenge for the obstetrician; ideally, preconception counseling and cardiology assessment should be offered and if this is not possible, risk assessment should be carried out from the first prenatal control. The best treatment consists of oxygen therapy, vasodilating agents, and anticoagulant medications with the goal of reducing the percentage of poor maternal-fetal outcomes. It is vitally important to define the time of birth in order to take the necessary therapeutic measures to avoid complications. In addition, multidisciplinary management and an intensive care unit are required due to the high probability of complications associated with this pathology.

Resumen Introducción: La enfermedad cardiovascular (ECV) constituye la principal causa de mortalidad en mujeres; la preeclampsia (PE) y la diabetes mellitus gestacional (DMG) están asociadas a incremento en el riesgo de ECV. Objetivo: Evaluar el conocimiento de los médicos generales (MG) sobre complicaciones obstétricas asociadas a ECV. Métodos: Se envió a los MG un cuestionario electrónico anónimo basado en casos, diseñado para evaluar el entendimiento de la influencia de la historia obstétrica en el riesgo cardiovascular a largo plazo y el conocimiento general sobre riesgo de ECV. Resultados: La tasa de respuesta fue de 35 % (161/465). Los participantes reconocieron que la PE y la DMG son factores de riesgo para ECV (98 y 83 %, respectivamente) y reportaron las siguientes estrategias de tamizaje de ECV en mujeres con historial de PE y DMG: monitoreo de presión arterial (PE 100 %, DMG 46 %), cálculo de índice de masa corporal (PE 68 %, DMG 57 %), evaluación del perfil de lípidos (PE 71 %, DMG 57 %), hemoglobina glucosilada (PE 26 %, DMG 92 %) y glucosa en ayuno (PE 28 %, DMG 91 %). Conclusión: Las estrategias de tamizaje para identificar ECV en mujeres con antecedentes de PE y DMG reportadas por los MG fueron variables.


Abstract Introduction: Cardiovascular disease (CVD) is the leading cause of mortality in women; preeclampsia (PE) and gestational diabetes mellitus (GDM) are associated with an increased risk of CVD. Objective: To evaluate general practitioners (GP) knowledge about complicated pregnancies and their association with CVD. Methods: An anonymous case-based electronic questionnaire designed to assess the level of understanding on the influence of a history of pregnancy complications on long-term cardiovascular risk and general knowledge about CVD risk was sent to GPs. Results: The response rate was 35 % (161/465). The participants recognized that PE and GDM are risk factors for CVD (98 and 83 %, respectively), and reported the following CVD screening strategies in women with a history of PE and GDM: blood pressure monitoring (PE 100 %, GDM 46 %), body mass index calculation (PE 68 %, GDM 57 %), lipid profile evaluation (PE 71 %, GDM 57 %), glycated hemoglobin (PE 26 %, GDM 92 %), and fasting glucose (PE 28 %, GDM 91 %). Conclusion: GP-reported screening strategies to identify CVD in women with a history of PE and GDM were variable.

ABSTRACT

Las enfermedades cardiovasculares representan la mayor causa de morbimortalidad a nivel mundial. Si bien presenta un descenso en la población general, en las mujeres tiende a mantenerse estable la prevalencia de enfermedad coronaria. Varios factores propios de la mujer predisponen a que esto ocurra, incluyendo el embarazo, mediado tanto por los cambios hematológicos y cardiovasculares característicos de la gestación; como por patologías asociadas, principalmente trastornos hipertensivos del embarazo y diabetes gestacional. Su presencia se ha asociado fuertemente a la aparición a futuro de otras patologías de alto riesgo cardiovascular como hipertensión crónica, dislipidemia y diabetes mellitus. Dado el impacto que esto representa, se hace imperante la identificación de grupos de alto riesgo y la implementación de medidas preventivas, así como de diagnóstico precoz y tratamientos adecuados con el fin de disminuir complicaciones materno-fetales en las etapas perinatal y posparto.


Cardiovascular disease is the leading cause of morbidity and mortality worldwide. Although there is a decrease in general population, the prevalence of coronary heart disease remains stable in women. Several factors typical of womenkind predispose to cardiovascular disease, including pregnancy, mediated by hematological and cardiovascular changes characteristic of it; and by associated pathologies, mainly hypertensive disorders and diabetes. The presence of these diseases has been strongly associated with future presence of other conditions of high cardiovascular risk such as chronic hypertension, dyslipidemia and diabetes mellitus. Given this impact, the identification of high-risk groups and the implementation of preventive measures, as well as early diagnosis and adequate treatment in order to reduce both maternal and fetal complications in perinatal and postpartum stages becomes imperative.

ABSTRACT

Abstract Objective To assess maternal and perinatal outcomes in pregnancies after kidney transplantation in a tertiary center in Brazil. Methods Retrospective cohort of pregnancies in women with kidney transplantation at the Universidade Estadual de Campinas, from January 1995 until December 2017. Medical charts were reviewed, andmaternal and perinatal outcomes were described as means and frequencies. Renal function and blood pressure were evaluated during pregnancy and postpartum. Results A total of 22 women had at least 1 pregnancy during the considered timeinterval, and 3 of them had > 1 pregnancy, totalizing 25 pregnancies. The mean age at transplantation was of 24.6 ± 4.2 years old, and the mean time interval until pregnancy was of 67.8 ± 46.3months. Themost frequent complication during pregnancywas hypertension, which affected 11 (64.7%)women. The gestational age at delivery was 34.7 ± 4weeks, and 47% of these pregnancies were preterm (< 37 weeks). A total of 88.2% of the women delivered by cesarean section. Renal function, measured by serum creatinine, remained stable during pregnancy, and the systolic blood pressure increased significantly, while the diastolic blood pressure did not differ during pregnancy. Conclusion Pregnancy after kidney transplantation is a rare event. Pre-eclampsia and prematurity were frequent complications, and cesarean section rates were very high. A specialized antenatal and postpartum care with a multiprofessional approach and continuous monitoring of graft function are essential for the early diagnosis of complications and improved outcomes.


Resumo Objetivo Avaliar os resultados maternos e perinatais de gestações em mulheres transplantadas renais em um centro terciário no Brasil. Métodos Coorte retrospectiva de gestações entre mulheres transplantadas renais na Universidade Estadual de Campinas, de Janeiro de 1995 a Dezembro de 2017. Os prontuários médicos foram revisados, e os resultados maternos e perinatais foram descritos como médias e frequências. A função renal e a pressão arterial foram avaliadas durante a gravidez e o puerpério. Resultados Um total de 22 mulheres tiveram ao menos 1 gravidez durante o período avaliado, e 3 delas tiveram > 1 gestação, totalizado 25 gestações. A idade média no momento do transplante foi 24.6 ± 4.2 anos, e o tempo médio de intervalo até a gravidez foi de 67.8 ± 46.3 meses. A complicação mais frequente durante a gravidez foi a hipertensão, que acometeu 11 (64.7%) mulheres. A idade gestacional no parto foi de 34.7 ± 4 semanas, e 47% das gestações encerraram-se prematuramente (< 37 semanas). Umtotal de 88.2% das gestações terminou com uma cesárea. A função renal, avaliada pela creatinina sérica, permaneceu estável durante a gravidez, enquanto a pressão arterial sistólica aumentou significativamente. A pressão arterial diastólica não diferiu ao longo dos períodos avaliados. Conclusão Gestação após o transplante renal é um evento raro. Pré-eclâmpsia e prematuridade foram as complicações mais frequentes, e as taxas de cesárea foram muito altas. O cuidado multiprofissional no pré-natal e no puerpério e a constante monitoração da função do enxerto são fundamentais para diagnosticar precocemente complicações e melhorar os resultados.

ABSTRACT

Resumen En Chile, las mujeres en edad fértil y las embarazadas presentan una alta prevalencia de tabaquismo. El consumo de tabaco durante el embarazo tiene efectos prenatales (por ejemplo, aborto espontáneo, mortinatalidad) y es un factor de riesgo de morbilidad y mortalidad infantil. Todas las mujeres en edad fértil deben ser alentadas a dejar de fumar, y las mujeres que ya están embarazadas deben ser alentadas a dejar de fumar continuamente durante todo el embarazo, desde el momento más precoz posible hasta el período posterior al parto. Se presenta un conjunto de antecedentes y recomendaciones para la cesación de tabaquismo en embarazadas, basadas en guías internacionales sobre este tema.


In Chile, women of childbearing age and pregnant women have a high prevalence of smoking. Tobacco use during pregnancy has antenatal effects (spontaneous abortion, stillbirth) and it is a risk factor for infant morbidity and mortality. All women of childbearing age should be encouraged to quit smoking, and women who are already pregnant should be encouraged to stop smoking continuously throughout the pregnancy, from the earliest possible moment to the postpartum period. We present a set of background information and recommendations for smoking cessation in pregnant women, based on international guidelines on this topic.

ABSTRACT

La hemorragia intracraneal constituye una patología poco frecuente del embarazo y asociada a pobres resultados materno-fetales. Dentro de sus causas se encuentra, la preeclampsia severa, la cual aumenta la mortalidad en estas pacientes. Se presenta el caso de una gestante que acude al servicio de urgencias con síntomas neurológicos e hipertensión arterial, diagnosticándose preeclampsia severa, con realización de cesárea de urgencia. La tomografía de cráneo mostró hemorragia temporal izquierda, requiriendo manejo en cuidados intensivos con posterior estabilización y alta médica. La hemorragia intracraneal requiere un alto índice de sospecha, monitoreo intensivo y manejo multidisciplinario.


Intracranial hemorrhage is a rare complication during pregnancy, it is associated with poor maternal and fetal outcomes. Cerebral haemorrhage caused by pre-eclampsia increases mortality in these patients. We present the case of a pregnant woman who consulted to the service of urgencies with neurological symptoms and hypertension. Severe preeclampsia was diagnosed. She underwent successful emergent cesarean section. The brain scan showed a left temporal hemorrhage. She required intensive care management with subsequent stabilization and discharged from the hospital. Intracranial hemorrhage requires a high index of suspicion, intensive monitoring, and a multidisciplinary approach.

ABSTRACT

El síndrome de Marfán es una enfermedad autosómica dominante del tejido conectivo, multisistémica y pronóstico determinado por las complicaciones cardiovasculares. Se presenta una paciente de 32 años, multípara, con el diagnóstico conocido de cinco años, posterior a disección aórtica toraco-abdominal (Standford B) y embarazo actual no planificado. Es manejada con betabloqueadores y las imágenes seriadas de aorta muestran disección estable en el segmento entre subclavia izquierda e ilíaca derecha con 41,8 mm en su diámetro mayor, sin comprometer la irrigación de órganos abdominales. Ingresa a las semana 32 para evaluación multidisciplinaria y parto programado. El examen con resonancia nuclear magnética de columna lumbosacra evidenció ectasia dural marcada a nivel de raíz S2. La evaluación fetal demostró un crecimiento en percentil 20 con bienestar hemodinámico y ecocardiografía normal. Previa inducción de madurez pulmonar y con 34+3 semanas, se efectúa cesárea electiva bajo anestesia espinal continua, con nacimiento sin compresión del fondo uterino, esterilización tubaria y postoperatorio inmediato en Unidad Coronaria por 48 horas. La evolución materna es sin incidentes. El recién nacido presentó enterocolitis necrotizante con buena respuesta al tratamiento médico.


Marfan syndrome is a multysistemic an autosomal-dominant disorder of connective tissue and cardiovascular complications determine its prognosis. We present a 32 year-old patient diagnosed five years previously after thoraco-abdominal aneurysm Standford B, and a current unplanned pregnancy. She was treated with beta blockers and evaluated with serial images of the aorta showing a stable thoraco-abdominal aneurysm initiated at the left subclavian artery to the right iliac artery of 41.8 mm at maximum diameter. The patient was hospitalized at 32 weeks to be evaluated in a multidisciplinary team planned delivery. Nuclear Magnetic Resonance on maternal spine detected dural ectasia at S2 and fetal evaluation shows growing at 20 percentile, hemodinamical wellbeing and normal echocardiography. After corticosteroids at 34 +3 weeks a cesarean section with tubal sterilization is performed, with continuous spinal anesthesia, without uterus compression and inmediate postoperative care at the Coronary Unit for 48 hours with satisfactory maternal evolution. The newborn presented a necrotizing enterocolitis at 7 days improving with medical therapy.

ABSTRACT

A população de obesos entre as mulheres na menacme tem aumentado de maneira assustadora nos últimos anos. A consequência é o aumento da incidência de gestantes obesas com elevado risco cardiometabólico e sujeitas a intercorrências perigestacionais. A assistência pré-natal nesse grupo de gestantes é precária devido à falta de equipe multidisciplinar preparada e material adequado. O profissional que assiste a gestante obesa deve ter como objetivo principal prevenir e intervir em tempo hábil nas intercorrências às quais mãe e feto estão sujeitos durante o período perigestacional. Ele deve estar livre de preconceitos e tentativas exageradas e inadequadas de perda de peso durante a gestação da paciente. O alvo principal da abordagem multidisciplinar é o preparo da gestante para mudança de seu estilo de vida familiar sabendo que a obesidade é uma doença de transmissão interpessoal de hábitos. A associação de riscos potenciais para mãe e feto à obesidade é bem estabelecida, mas o conhecimento desses riscos e a condução da gestação de maneira adequada são pouco praticados. O presente artigo buscou contribuir para melhora da assistência pré-natal das gestantes obesas juntamente com a criação de grupos multidisciplinares para condução dessas pacientes.


The obese population between women in the mecname has been grown in a concerned manner in the past few years and its consequence is the growth of the obese pregnant patients with high cardio metabolic risk and subject to clinical intercurrences. The prenatal assistance in this pregnant group is precarious due to a lack of a prepared multidisciplinary group and also an adequate work material. The professional whom takes care of an obese patient shall be as the main purpose the prevention and intervention in a timely manner in the clinical intercurrences which mother and fetus are subject to during the gestational period. His method shall be free of prejudices, inadequate and exaggerated attempts to loss of weight during the patient pregnancy. The main target of the multidisciplinary approach is the pregnant patient's prepare to a new familiar lifestyle keeping in mind that obesity is a sickness of interpersonal habit transmission. The association of potential risks to mother and fetus during obesity is well determined but the knowledge of those risks and the pregnancy conduction in a proper manner are few practiced. The present article wanted to contribute to an improvement of prenatal assistance of obese pregnant jointly with a multidisciplinary group creation to conduct those patients.

ABSTRACT

Tromboembolismo pulmonar e trombose venosa profunda são os doiscomponentes da síndrome de tromboembolismo venoso, atualmente considerada uma das principais causasde morbimortalidade materna no mundo ocidental. Durante a gestação, classicamente conhecida como umestado de hipercoagulação, a frequência de fenômenos tromboembólicos é ainda maior. A presença de fatoresde risco para o tromboembolismo adicionada à gravidez requer atenção especial, visto que diagnóstico tardio,tratamento inadequado ou tardio e profilaxia imprópria podem levar à morte materna. Esta revisão tem comoobjetivo esclarecer os fatores de risco relacionados a fenômenos tromboembólicos na gestação, os métodos dediagnóstico disponíveis, medidas de profilaxia e tratamentos atualmente recomendados.


Pulmonary thromboembolism and deep vein thrombosis are the twocomponents of Venous Thromboembolism Syndrome, considered nowadays as one of the leading causes ofmaternal morbimortality in the occidental world. During the gestational period, which is classically known as ahipercoagulable state, the frequency of thromboembolic events is even bigger. The presence of risk factors forthromboembolism added to pregnancy demands special attention, since late diagnosis, inadequate or delayedtreatment and improper prophylaxis may lead to maternal death. The purpose of this review is to present the riskfactors related to thromboembolic events in the gestational period, to discuss available diagnostic and prophylaxismethods and treatments currently recommended.

ABSTRACT

A ocorrência de Hemopericárdio com Tamponamento Cardíaco é uma complicação da Valvoplastia Mitral Percutânea por balão (VMB), que apesar de pouco freqüente, pode ter graves conseqüências materno-fetais. Este artigo descreve o caso de um tamponamento cardíaco após VMB em uma mulher de 28 anos, com estenose mitral reumática severa na 20ª semana de gravidez.


The occurrence of hemopericardium with cardiac tamponade is a complication of percutaneous balloon mitral valvuloplasty (BMV) which, although uncommon, may have severe maternal and fetal consequences. This article describes a case of cardiac tamponade following BMV in a 28-year-old woman with severe rheumatic mitral stenosis in the 20th week of gestation.

El objetivo de la investigación fue comparar las concentraciones plasmáticas de homocisteína y perfil lipídico en preeclámpticas y controles además establecer la relación entre la homocisteína y las concentraciones de colesterol, triglicéridos y lipoproteínas. Setenta pacientes fueron seleccionadas y divididas en dos grupos, preeclámpticas (grupo A) y embarazadas normotensas (grupo B), consideradas como controles, estudiadas en forma consecutiva. Se midieron las concentraciones de colesterol total, triglicéridos, lipoproteínas de alta densidad (HDL- C), baja densidad (LDL-C), muy baja densidad (VLDL) y homocisteína. Se encontraron diferencias estadísticamente significativas en las concentraciones de triglicéridos, HDL-C, VLDL, y homocisteína (P < 0,05). No se encontraron diferencias significativas en las concentraciones plasmáticas de colesterol y LDL-C (p=ns). Al analizar exclu ivamente al grupo de las preeclámpticas se encontró que la única correlación significativa y positiva fue observada entre la homocisteína y triglicéridos (r = 0,229; P < 0,05). Se concluye que existen diferencias significativas en las concentraciones de triglicéridos, H D L-C, V LDL y homocisteína entre las preeclámpticas y los controles normotensas. Además, existe una correlación positiva y significativa entre las concentraciones de homocisteína y de triglicéridos en las preeclámpticas


The objective of the research was to compare plasma concentrations of homocysteine and lipid profile in preeclamptic and control patients and establish the relation between homocysteine and cholesterol, triglycerides and lipoprotein concentrations. Seventy patients were selected and divided in two groups, preeclamptic (group A) and normotensive pregnant women (group B) considered as controls, studied in consecutive way. Concentrations of total cholesterol, triglycerides, high-density (HDL-C), low-density (LDL-C), very-low density lipoproteins (VLDL) and homocysteine were measured. There were significant statically differences in triglycerides, HDL-C, VLDL and homocysteine (P < 0.05). There were no significant differences in cholesterol and LDL-C concentrations. When preeclamptic group was analyzed exclusively, there were observed only significant and positive correlation between homocysteine and triglycerides (r=0.229; P < 0.05). It is concluded that there are significant differences in triglycerides, HDL-C, VLDL and homocysteine between preeclamptic and normotensive controls. More over, there is a positive a significant correlation between homocysteine and triglycerides in preeclamptic patients

FUNDAMENTO: Os fatores de risco para as síndromes hipertensivas gestacionais (SHG) são vários e podem relacionar-se à região e etnia da população. No Estado de Goiás, são escassos os estudos sobre esse tema. OBJETIVO: Investigar os fatores de risco maternos para SHG. MÉTODOS: Utilizou-se um estudo caso-controle por meio da análise dos prontuários das parturientes da Maternidade do Hospital das Clínicas da Universidade Federal de Goiás (HC-UFG), em 2005. Os fatores de risco foram analisados pela análise de regressão logística e pelo teste exato de Fisher. RESULTADOS: Em 2005, houve 890 partos na Maternidade do HC-UFG, e 129 gestantes apresentaram diagnóstico de SHG (14,5 por cento). A análise multivariada identificou a obesidade como fator de risco tanto para hipertensão gestacional (HG) (OR: 17,636; IC 95 por cento: de 2,859 a 108,774) como para hipertensão arterial crônica superajuntada à pré-eclâmpsia (HCSPE) (OR: 27,307; IC 95 por cento: de 4,453 a 167,440). Primiparidade constituiu fator de risco para HG (OR: 5,435; IC 95 por cento: de 1,923 a 15,385). Idade acima de 30 anos foi fator de risco para HCSPE (OR: 5,218; IC 95 por cento: de 1,873 a 14,536) e fator de proteção para pré-eclâmpsia (PE) (OR: 0,035; IC 95 por cento: de 0,003 a 0,364). Raça não-branca representou risco independente para PE (OR: 13,158; IC 95 por cento: de 1,672 a 100,000) e PE prévia para HCSPE (OR: 4,757; IC 95 por cento: de 1,261 a 17,938). Das gestantes com hipertensão arterial crônica (HAC), 73,5 por cento desenvolveram HCSPE (p < 0,001). CONCLUSÃO: Os fatores obesidade, raça não-branca, PE prévia, idade acima de 30 anos e HAC identificados foram semelhantes à maioria dos achados da literatura.


BACKGROUND: There are several risk factors for hypertensive syndromes in pregnancy (HSP), and these can be related to regional and ethnic factors. Studies on this issue are scarce in the State of Goiás. OBJECTIVE: To investigate maternal risk factors for HSP. METHODS: Case-control study based on the analysis of medical records of parturients from the Maternity Service of Hospital das Clínicas da Universidade Federal de Goiás (HC-UFG) in 2005. Risk factors were analyzed using Logistic Regression Analysis and Fisher's Exact Test. RESULTS: In 2005, there were 890 deliveries in the Maternity Service of HC-UFG, and 129 pregnant women were diagnosed with HSP (14.5 percent). The multivariate analysis identified obesity as a risk factor both for gestational hypertension - GH (OR: 17.636; 95 percent CI: 2.859 to 108.774) and for preeclampsia superimposed on chronic hypertension - PESCH (OR: 27.307; 95 percent CI: 4.453 to 167.440). Primiparity was a risk factor for GH (OR: 5.435; 95 percent CI: 1.923 to 15.385). Age above 30 years was a risk factor for PESCH (OR: 5.218; 95 percent CI: 1.873 to 14.536) and a protective factor against preeclampsia - PE (OR: 0.035; 95 percent CI: 0.003 to 0.364). Non-white race was an independent risk for PE (OR: 13.158; 95 percent CI: 1.672 to 100.000) and previous PE for PESCH (OR: 4.757; 95 percent CI: 1.261 to 17.938). Of the pregnant women with chronic hypertension (CH), 73.5 percent developed PESCH (p < 0.001). CONCLUSION: The factors identified - obesity, non-white race, previous PE, age above 30 years and CH, were similar to those found in the majority of studies in the literature.

ABSTRACT

The socio-demographic, behavioral and anthropometric correlates of C-reactive protein levels were examined in a representative young adult Brazilian population. The 1982 Pelotas Birth Cohort Study (Brazil) recruited over 99 percent of births in the city of Pelotas that year (N = 5914). Individuals belonging to the cohort have been prospectively followed up. In 2004-2005, 77.4 percent of the cohort was traced, members were interviewed and 3827 individuals donated blood. Analyses of the outcome were based on a conceptual model that differentiated confounders from potential mediators. The following independent variables were studied in relation to levels of C-reactive protein in sex-stratified analyses: skin color, age, family income, education, parity, body mass index, waist circumference, smoking, fat/fiber/alcohol intake, physical activity, and minor psychiatric disorder. Geometric mean (95 percent confidence interval) C-reactive protein levels for the 1919 males and 1908 females were 0.89 (0.84-0.94) and 1.96 mg/L (1.85-2.09), respectively. Pregnant women and those using oral contraceptive therapies presented the highest C-reactive protein levels and all sub-groups of women had higher levels than men (P < 0.001). Significant associations between C-reactive protein levels were observed with age, socioeconomic indicators, obesity status, smoking, fat and alcohol intake, and minor psychiatric disorder. Associations were stronger at higher levels of C-reactive protein and some associations were sex-specific. We conclude that both distal (socio-demographic) and proximal (anthropometric and behavioral) factors exert strong effects on C-reactive protein levels and that the former are mediated to some degree by the latter.

ABSTRACT

No definitive recommendation is available concerning optimal antithrombotic therapy in pregnant women with a mechanical heart valve. The purpose of the current study was to evaluate the clinical results of nadroparin treatment with respect to pregnancy outcome and maternal complications. From 1997 to 2005, 31 pregnancies were reviewed in 25 women. Nadroparin (7,500 U, twice daily) was used in 23 pregnancies between 6 and 12 weeks of gestation and close-to-term only, and coumarin derivatives were used with aspirin at other times. Eight pregnant women treated with coumarin derivatives throughout pregnancy were compared to evaluate the safety and efficacy of nadroparin. No maternal death or bleeding complication occurred in either of the two groups, and frequencies of maternal thromboembolism including valve thrombosis (8.7% vs. 12.5%, p>0.05) were similar. However, the frequencies of live born (91.3% vs. 50%, p=0.01) and healthy babies (90.4% vs. 25%, p<0.01) were significantly higher, and the fetal loss rate was significantly lower (8.7% vs. 50%, p=0.01) in the nadroparin-treated group. Regarding the efficacy and safety of antithrombotic treatment in pregnant women with prosthetic heart valves, nadroparin treatment during the first trimester is an acceptable regimen and produces better results than coumarin derivatives.

OBJETIVO: Quantificar a prevalência de HG em adolescentes primíparas; definir fatores de preditividade para a ocorrência de HG e sua repercussão nos recém-nascidos. MÉTODOS: Foram acompanhadas 29 adolescentes primíparas durante o pré-natal até a 12ª semana de puerpério, com idade média de dezesseis anos, do ambulatório do Serviço de Adolescentes da Maternidade Escola Assis Chateaubriand (MEAC) da Universidade Federal do Ceará (Fortaleza). As gestantes foram dividas em dois grupos; ou seja, nas que permaneceram normotensas (Grupo I) e naquelas que desenvolveram hipertensão gestacional -HG - (Grupo II). As variáveis investigadas na avaliação de valor de preditividade no desenvolvimento de HG foram valores antropométricos, aspectos socioeconômicos, o hábito de fumar, a hereditariedade para HAS (pai/mãe), os exames do pré-natal solicitados na primeira consulta do pré-natal e a microalbuminúria e a monitorização ambulatorial da pressão arterial (MAPA) na 28ª semana de gestação. As gestantes foram acompanhadas no parto e no puerpério tardio (12ª semana de puerpério). Os recém-nascidos das mães do nosso estudo foram avaliados ao nascer pelos índices de APGAR e de Capurro, peso, estatura e pela presença de hipóxia perinatal. RESULTADOS: A prevalência de HG foi de 51,7 por cento . A hereditariedade para HAS apresentou-se com o maior valor de preditividade para HG com odds ratio de 10,99. A pressão arterial diastólica maior ou igual a 70 mmHg, na idade média gestacional de 35 semanas, apresentou significância estatística como valor de preditividade para HG. Na MAPA encontramos valor de preditividade para HG: carga pressórica diastólica em vigília, carga pressórica sistólica e diastólica no sono noturno, variabilidade pressórica e pressão máxima diastólica no sono. Especificamente a pressão arterial diastólica (PAD) máxima na MAPA no período do sono noturno ³64 mmHg apresentou odds ratio de 6 para HG com sensibilidade de 80 por cento e especificidade de 60 por cento para o desenvolvimento de HG...


OBJECTIVE: To quantify PH prevalence in primiparous adolescents; define predictive factors for the occurrence of PH and its impact on newborns. METHODS: We followed 29 primiparous adolescents from the prenatal period through the 12th week of the puerperium, with a mean of sixteen years of age, served at the Outpatient Facility for Adolescents of Maternidade Escola Assis Chateaubriand (MEAC) of Universidade Federal do Ceará (Fortaleza, Brazil). The pregnant adolescents were divided into two groups, that is, those who remained normotensive (Group I) and those who developed PH (Group II). The variables investigated in the assessment of the value of predictability for the development of PH were anthropometric measures, socioeconomic aspects, smoking habit, inheritance for SAH (father/mother), prenatal tests requested in the first prenatal care visit in addition to microalbuminuria and ambulatory blood pressure monitoring (ABPM) in the 28th week of gestation. The pregnant adolescents were followed up at delivery and late puerperium (12th week after the puerperium). The newborns to the mothers included in our study were assessed at birth according to the Apgar score and the Capurro method, for weight, height and perinatal hypoxia. RESULTS: The prevalence of PH was 51.7 percent. Inheritance for SAH presented the highest predictive value for PH with an odds ratio of 10.99. Diastolic arterial pressure equal to or above 70 mmHg at the gestational age of 35 weeks was statistically significant as a predictive value for PH. At ABPM we found a predictive value for PH: diastolic pressure load during alertness, diastolic and systolic pressure load during night sleep, pressure variability and maximum diastolic pressure during sleep. ABSTRACT

Peripartum cardiomyopathy (PPCM) is a condition that affects women during the reproductive years in the late pregnancy period and/or early postpartum period. Although it is associated with several risk factors and various hypotheses exist of its etiology the cause of this disorder is still unknown. Standard therapy for PPCM is the same as for heart failure. Studies examining new therapeutic approaches are adding to the armamentarium available to physicians treating patients with PPCM. Despite all the current knowledge the mortality rates associated with PPCM remain relatively high. This article is a review of the current knowledge of etiology, diagnosis, treatment and prognosis of PPCM and attempts to present areas of need of further research.

Foram estudados 571 partos de mulheres cardiopatas (T) e seus subgrupos: arritmias (A), congênitas (CC) e adquiridas (CA), para avaliar o tipo de parto, os fatores clínicos e obstétricos relacionados à determinação da via de parto e suas complicações. Foi verificado que as taxas de cesárea foram elevadas (T=57,2 por cento, A=45,6 por cento, CC=64,2 por cento, CA=55,7 por cento), e estiveram relacionadas à presença de: cesárea anterior (T), intercorrências obstétricas (T, CC e CA), idade gestacional no parto menor que 37 semanas (T, CA), insuficiência cardíaca (T, CC), medicamentos cardiovasculares (T, A) e cardiopatia congênita (T). Complicações obstétricas foram mais freqüentes em partos vaginais e as clínicas em cesáreas / The mode of delivery was studied in 571 pregnancies in cardiac women (T) in order to establish the clinical and obstetrical factors related to the kind of delivery and their complications. The results showed that cesarean rates were high (total=57,2 per cent, arrhythmias A=45,6 per cent, congenital diseases CD=64,2 per cent, and acquired diseases AD=55,7% per cent and were related to: previous cesarean sections (T), obstetrical events (T, CD, AD), prematurity (T, AD), heart failure (T, AD), cardiovascular drugs (T, A) and congenital heart disease (T).
